Organizations face challenges in seeking better health and safety outcomes for their workforce, particularly in reducing incidents involving injury accidents. From this scenario, companies undertake efforts and actions to mitigate these undesirable events, however, without being sure of the efficiency of the labor spent. For this reason, this monograph presents the application of root cause analysis of occupational accidents within the electric utility of the state of Paraná from the application of reliability engineering tools and administration. The tools of the Ishikawa Diagram combined with the 5 Whys technique are used to identify accidentgenerating situations and their respective root causes, as well as quality tools to record and monitor proposed actions. In order to analyze and verify the trends, improvements and worsening of the process, as well as to predict its evolution from the implemented actions, the reliability engineering method called monitored growth of reliability was used. Finally, the hierarchical process analysis technique was applied in order to prioritize the implementation of the proposed mitigating actions. The work identified the main situations that generate accidents of the organization and their respective root causes, with the proposition of preventive actions and mitigating occupational accidents, as well as allowing the monitoring of the evolution of actions implemented through the monitored growth of reliability. |
